Monopolar technology (old-fashioned “radiosurgery”) cuts by advancing a spark, and must never allow contact with bone or metal as osseonecrosis can result. Monopolar units cannot cut or coagulate under water, and thus, employ very high voltage to achieve fulguration, inducing a spark to cook surrounding tissue. Lasers, of course, work entirely by destroying tissue with heat.
